Sweetwater Medical Associates looks forward to having you join our host of satisfied patients. Our appointments are flexible and tailored for your convenience. We see patients by appointment only, Monday through Friday from 8:30 AM to Noon and from 2 PM to 5 PM.

 

To schedule an appointment, please call us at (281) 494-4900, ext. 201. To request an appointment online for an annual physical examination or a non-urgent follow up appointment, please complete and submit our Online Appointment Request Form.

 

 

Once you have made your initial appointment, you can pre-register online by simply clicking on the links below. Please complete the forms and click submit. Alternatively, you may download the forms, print them, fill them out, and bring them with you to your visit. Completing the forms prior to your visit will help speed up the new patient registration process and get you in to see the doctor faster!
